Enrolling as a MI Choice Service Provider with TCOA

Tri-County Office on Aging (TCOA), the Region VI Area Agency on Aging for the State of Michigan, administers the MI Choice Waiver program in Clinton, Eaton, and Ingham counties. TCOA helps older adults, adults with disabilities, and caregivers access programs and benefits, improve their health, and remain active in their communities.

Working in partnership with agencies and service providers across the region, TCOA embraces a person-centered philosophy that focuses on how a person wants to live their life while honoring their preferences. The MI Choice Waiver allows these individuals to remain in the comfort of their homes or residential facilities while receiving essential services to meet their needs. Eligible adults who meet level of care, income, and asset criteria can access MI Choice Waiver Medicaid-covered services, delivered via our network of contracted service providers.

If your agency or business is interested in becoming a contracted service provider with TCOA, you will need to (1) Meet the MI Choice Waiver program requirements, and (2) complete all TCOA Provider Enrollment and Contracting Documentation.
